Service Report Template for Maintenance and Field Service Work

A free service report template for maintenance and field service teams: every field a good report needs, a filled-in sample, variants for aircon, CCTV and fire protection work, and the mistakes that cause disputes.

Direct Answer

A good service report records who did what, where, on which equipment, what was found, what was done, what is still outstanding, and who confirmed it. The template below has every field in that order. Copy it into Word, Excel, Google Sheets or a form tool, and add any fields your contracts require. When writing reports by hand starts costing your team an hour a day, the same fields are what a service report app fills in for them.

The Template

Copy this table and fill in the right-hand column.

Section Field What to write
Header Company name and logo Your registered name
Report number A unique number, never reused
Work order or job reference The number the customer's PO or your job uses
Date of service The day the work was done, not the day the report was typed
Customer and site Customer name The company being billed
Site name and address Where the work was done, including block and unit
Site contact Name and phone of the person on site
Equipment System or equipment For example: split aircon unit, CCTV recorder, fire alarm panel
Location on site Room, floor, riser or zone
Make, model, serial or tag number So the next visit can find the same unit
Work Type of visit Preventive maintenance, corrective repair, inspection, installation, or call-out
Reported problem In the customer's words, if it was a call-out
Checks performed One line per check, with the result
Work done What was actually done, in plain words
Parts and materials used Item, quantity, and whether it is chargeable
Time on site Arrival and departure times
Findings Defects found What is wrong and still not fixed
Recommendations What should be done, and how urgent it is
Photos Before and after, labelled to the item they show
Next service due Date, if the work is on a schedule
Sign-off Technician name and signature The person who did the work
Customer name, signature and date The person who accepted the work on site
Remarks Anything the customer asked to record

A Filled-In Sample

An illustrative example, not a real job.

  • Report number: SR-2026-0412. Job reference: WO-2026-0388. Date of service: 21 October 2026.
  • Customer: Example Logistics Pte Ltd. Site: Warehouse 3, 12 Example Road. Site contact: Operations supervisor, 9XXX XXXX.
  • Equipment: Split aircon unit, office level 2, tag AC-07.
  • Type of visit: Preventive maintenance, quarterly.
  • Checks performed: Filters cleaned. Drain pipe flushed, flowing. Refrigerant pressure within the maker's range. Fan coil cleaned.
  • Work done: General servicing, no parts replaced.
  • Time on site: 09:10 to 09:55.
  • Defects found: Insulation on the outdoor pipe is torn for about 30 cm.
  • Recommendation: Replace pipe insulation at the next visit. Not urgent.
  • Photos: 4, labelled "filter before", "filter after", "drain outlet", "torn insulation".
  • Next service due: January 2027.
  • Signed: Technician, and the site supervisor, 21 October 2026.

Variants by Trade

Keep the same structure and change the equipment and checks sections.

  • Aircon servicing: unit tag and location per unit, filter, drain, coil and refrigerant checks, and one row per unit when a visit covers several.
  • CCTV: camera number and location, image quality, recording retention checked, recorder health and storage, time sync.
  • Fire protection: the system serviced (alarm, sprinkler, extinguishers, hose reels and so on), the devices or zones tested, defects with their location, and the servicing standard or contract the visit was done under. Fire protection companies usually need reports that follow their market's servicing standard, so check what your contract and your authority require.
  • General facilities maintenance: asset tag, task from the maintenance schedule, condition rating, and follow-up work order number if one was raised.

Mistakes That Cause Disputes

  • No equipment identifier. Nobody can prove which unit was serviced.
  • The date the report was typed, not the date of the work. It breaks the service history.
  • "Checked OK" with no detail. It cannot be defended when the item fails next month.
  • Defects buried in remarks. A defect needs its own line so it turns into a quotation or follow-up job.
  • No customer sign-off. The visit is hard to bill and easy to dispute.
  • Photos with no label. Forty unlabelled photos prove nothing.

When the Template Stops Being Enough

A template on paper or in a spreadsheet works until the volume grows. The signs: reports typed up in the evening, photos sitting in WhatsApp, defects that never become quotations, and invoices waiting on reports. At that point the same fields belong in an app the technician fills in on site, which sends the report and feeds the defects and the billing. See service report automation and the field service platform. For fire protection companies, Lyt Brox builds Firavia, an operations system for that trade.

FAQ

What should a service report include? The job and report numbers, date of service, customer and site, the equipment serviced with its identifier, the type of visit, checks and work done, parts used, time on site, defects and recommendations, photos, the next service date, and both the technician's and the customer's sign-off.

What is the format of a service report? Header, customer and site, equipment, work done, findings, then sign-off, in that order, as in the template on this page.

Is there a free service report template? Yes. The template on this page is free to copy into Word, Excel, Google Sheets or a form tool.

What is the difference between a service report and a work order? A work order is the instruction to do the job. A service report is the record of what was actually done, found and signed for. Each report should carry its work order number.

How do I make service reports faster? Fill them in on site, on a phone, instead of typing them up later. A service report app does this with photos attached and the report sent automatically.

Summary

Use the template above: header, customer and site, equipment, work, findings, sign-off. Record the equipment identifier and the date of the work, give every defect its own line, label every photo, and get the customer's signature. When reports start being typed up after hours, move the same fields into an app.
